Does Creatine Actually Cause Hair Loss? What the Research Says
Creatine is one of the most widely used and researched sports supplements available, valued for supporting muscle strength and exercise performance. But a single study from 2009 continues to fuel an ongoing debate in hair loss communities: does creatine accelerate hair shedding by raising DHT, the hormone most closely linked to androgenetic alopecia (pattern hair loss)?
Where the Concern Started
The concern traces back to a small study of college rugby players, which found that three weeks of creatine supplementation was associated with an increase in dihydrotestosterone (DHT) levels and in the ratio of DHT to testosterone. Because elevated DHT is a well-established driver of androgenetic alopecia in genetically predisposed people, this finding was enough to raise a flag, even though the study did not actually measure hair shedding or hair loss as an outcome.
What Later Research Has (and Hasn’t) Shown
Since that original study, the picture has stayed murkier than a lot of online discussion suggests. According to a review summarized by the Harvard T.H. Chan School of Public Health, creatine is generally considered safe for most healthy adults at standard doses, and the broader body of research on its effects has focused mainly on muscle performance, kidney function, and hydration rather than hair-specific outcomes.
Importantly, no large, controlled study has directly demonstrated that creatine supplementation causes measurable hair shedding or accelerates pattern baldness in humans. The 2009 finding on DHT has not been consistently replicated in follow-up research, and DHT elevation alone doesn’t automatically translate into visible hair loss for everyone — genetic sensitivity of hair follicles to DHT (androgen receptor sensitivity) appears to matter more than DHT levels in isolation, based on general dermatological understanding of androgenetic alopecia.
What This Means If You’re Already on a Hair Loss Regimen
For people already using DHT-blocking treatments such as finasteride or dutasteride, or topical minoxidil, the theoretical concern about creatine-driven DHT spikes is at least partially addressed by the mechanism of those medications, since 5-alpha-reductase inhibitors work specifically by blocking the conversion of testosterone to DHT. That said, this is a reasonable inference from how the medications work, not a claim that’s been specifically tested in creatine users.
If you’re noticing new shedding after starting creatine, it’s worth remembering that hair shedding has many possible causes, including stress, diet changes, illness, and the natural fluctuations of the hair growth cycle, and telogen effluvium (temporary, diffuse shedding) is common and often unrelated to supplements. The American Academy of Dermatology notes that some degree of daily hair shedding is normal, and a noticeable change is worth discussing with a dermatologist rather than attributing to any one variable without evaluation.
The Bottom Line
The creatine-and-hair-loss link remains an open question rather than a settled one. The available evidence doesn’t currently support a strong causal claim in either direction, and anyone concerned about hair thinning, whether or not they take creatine, is generally better served by tracking changes over time and discussing them with a dermatologist who can evaluate hormone levels, family history, and other contributing factors.
